Before anything else

Two checks, ten minutes, and they resolve the problem entirely for a lot of people.

Are you eligible for coverage? New York’s income limits are broader than most states’. Medicaid, the Essential Plan and Child Health Plus have no enrollment season. Immigration status disqualifies fewer people here than most assume. The ten-minute eligibility check, or the 2026 income limits by household size.

Are you 65 or older? Then EPIC is a state program you can join in any month, with no documents attached to the application, that holds your copay to no more than $20 per drug. How EPIC works.

If neither applies, the rest of this page is the method.

The question that costs nothing to ask

“What does this cost if I don’t run it through insurance?”

Ask it even if you have coverage. For a number of common generics, the cash or discount-card price sits below the plan’s copay, and the pharmacy is not obliged to point that out. Ask it every time the price surprises you.

The three routes, and what each is for

Discount programs and cards. Free to use, work best on generics, and are accepted at most chain pharmacies. They are not insurance and they do not count toward any deductible. The price varies by pharmacy for the same card, so the card and the pharmacy are two separate decisions.

Manufacturer patient assistance. For brand-name drugs, most manufacturers run a program that provides the medication free or at a fixed low price to people under an income threshold. You apply directly with the manufacturer, usually with the prescriber’s signature on part of the form. These are underused because the application looks intimidating and the thresholds are higher than people expect.

340B pharmacies at community health centers. Federally qualified health centers participate in a federal drug pricing program and can dispense to their own patients at deeply discounted prices. This is often the cheapest route available, and it requires becoming a patient of the health center, which is itself worth doing since they also charge for visits on a sliding scale by income. Search by ZIP at findahealthcenter.hrsa.gov.

Talk to the prescriber before you shop

The cheapest version of this problem is often solved in the exam room rather than at the counter.

Ask whether a generic or an older equivalent would work. Sometimes the answer is no. Often nobody asked.

Ask about a 90-day supply. Frequently cheaper per dose, and one dispensing fee instead of three.

Say the number out loud. Prescribers are not told what their patients pay, and a specific figure changes prescribing more reliably than a general statement that money is tight.

Ask for samples for a new medication you are not yet sure you will stay on.

What to expect from prices

We are not publishing dollar figures for specific drugs on this page, and the reason is not caution for its own sake. Prices for the same prescription differ by pharmacy, by discount program, by month and by dose. A number here would be wrong for most readers within weeks, and a wrong number is worse than no number when someone is deciding whether they can afford a refill.

What we can say with confidence is that the variation is large enough to be worth three phone calls, and that the cash price and the insured price are two different things that must be asked about separately.

When it is a hospital or clinic bill, not a pharmacy one

Drugs administered in a hospital or a hospital-owned clinic are billed as part of that visit, not through a pharmacy. That puts them inside New York’s hospital financial assistance law: nothing out of pocket at or below 200% of the federal poverty level, capped at 10% of the Medicaid rate to 300% and 20% to 400%, no asset test, immigration status not considered, and applications accepted even after a bill goes to collections.

Nobody at the counter will mention it. Ask for the hospital’s financial assistance application by name.

Questions people ask

How do I get cheaper prescriptions without insurance?

Ask the cash price at the counter, use a discount program for generics, apply to manufacturer assistance for brand drugs, and get your prescriptions from a 340B pharmacy at a community health center if you can become a patient there.

Is the cash price ever lower than my copay?

For a number of generics, yes. Ask at every counter, since the pharmacy is not required to tell you.

What is 340B?

A federal drug pricing program that lets participating providers, including federally qualified health centers, dispense to their patients at heavily discounted prices.

What is patient assistance?

A manufacturer program that supplies a brand-name drug free or at a low fixed price to people under an income threshold. You apply to the manufacturer, usually with the prescriber’s signature.

Do discount cards work with insurance?

They are used instead of insurance, not alongside it, and what you pay does not count toward a deductible. For a cheap generic that is often still the better deal.

I am over 65 in New York. Is there a state program?

Yes, EPIC. Income limits reach $75,000 single and $100,000 for a couple, applications are taken any month, and copays are capped at $20 per drug.
