The three parts of a routine visit

The examination. The dentist’s assessment.

The cleaning. And this is the line where people are surprised: a routine cleaning and a deep cleaning (scaling and root planing) are different services at different prices, and the second is often recommended at a first visit. It may well be justified. It is also not what you booked, so ask before it happens:

“Is this a routine cleaning or a deep cleaning, and what does the difference cost?”

X-rays. Their own codes, priced by how many and which type. A full series at a first visit costs more than routine bitewings.

The annual maximum, and why coverage runs out

A medical plan has an out-of-pocket maximum: a ceiling on what you pay, after which the plan covers everything. A dental plan usually has an annual maximum: a ceiling on what the plan pays, after which you cover everything.

Those are opposite mechanisms with similar names, and confusing them is how people end up mid-treatment with no benefit left.

If you have Medicaid

New York Medicaid covers dental, and the rules changed substantially on January 31, 2024 under a class action settlement: root canals are covered when medically necessary, crowns are no longer blocked by the old four-pairs-of-teeth rule, implants are covered in many cases, and replacement dentures no longer need a letter from your physician.

The most useful consequence: a denial issued before that date was decided under criteria that no longer exist. If you were told no, ask again. The full picture.

Children’s dental coverage on Medicaid and Child Health Plus is broader than adult coverage, and in New York children’s coverage has no immigration requirement.

If you have no dental coverage

Federally qualified health centers with dental services charge on a sliding scale by income and treat uninsured patients as routine. Search by ZIP at findahealthcenter.hrsa.gov. This is usually the fastest real option.

Dental school clinics treat at reduced fees under faculty supervision. Appointments are longer, waits are real, the supervision is genuine.

Ask any practice for the self-pay rate, which is frequently below the charge submitted to a plan, and ask for a written treatment plan with codes and prices before agreeing to anything beyond an exam.

A hospital dental clinic bill falls under New York’s hospital financial assistance law if the hospital does the billing, which almost nobody realises. Ask for the financial assistance application by name.

When dental is billed as medical

Some oral procedures are billed through medical insurance rather than dental: certain surgical extractions, jaw procedures, treatment after facial trauma, and hospital-based dental care for patients who need general anaesthesia.

If a procedure is significant, ask which one it goes through. The answer changes the deductible, the network and the annual maximum that applies.

Questions people ask

How much does a dental cleaning cost?

It depends on whether it is a routine cleaning or a deep cleaning, and on whether X-rays and an exam are billed alongside. Ask for the treatment plan with codes.

What is an annual maximum?

The most a dental plan will pay in a year. Unlike a medical out-of-pocket maximum, it caps what the plan pays, not what you pay.

Does New York Medicaid cover root canals?

Yes, when medically necessary, since January 31, 2024. If you were denied before that date, the answer was made under criteria that no longer exist.

Where can I get dental care without insurance?

Community health centers with dental services charge on a sliding scale by income, and dental school clinics charge reduced fees under supervision.

Why is a deep cleaning more expensive?

It is a different, more extensive service with its own codes. It may be appropriate, and it is worth asking about before it happens rather than after.

Can dental work be billed to medical insurance?

Sometimes, for surgical, trauma-related or hospital-based dental care. Ask which system a significant procedure goes through.
