The question at the desk
Urgent care and hospital emergency departments are different regulatory animals that increasingly look the same from the street: same block, same glass front, same hours on the door.
The billing consequences are not similar at all.
| What it is | What you are billed |
|---|---|
| Independent urgent care center | A visit charge, plus each test or procedure. Often a published self-pay rate. |
| Hospital-affiliated urgent care | Visit, plus procedures, plus a facility fee because the site is licensed as part of the hospital. |
| Freestanding emergency department | Billed as an emergency room visit, which is a different and much larger category. |
Ask it plainly, before registration, because after registration you have accepted the terms:
“Is this an urgent care center or a hospital emergency department? Do you bill a facility fee?”
What the visit charge does and does not include
The advertised price, where one exists, is generally the visit itself. Everything performed during it is separate:
- Rapid tests for strep, flu, COVID, mono
- X-ray, and the radiologist’s read as a separate line
- Stitches, splinting, wound care
- Injections and medications administered on site
- Any specimen sent to an outside lab, which bills you directly weeks later
That last one catches people. Ask specifically: “If anything is sent out to a lab, who bills me for that?”
When urgent care is the wrong door
Cost is not the only variable, and this is the place to be blunt about it. Urgent care is built for things that need attention today and are not dangerous. Chest pain, difficulty breathing, signs of stroke, severe bleeding, a serious head injury, and anything you would describe as an emergency belong in an emergency room, and the money question comes afterward.
The reverse case matters too: an emergency room visit for something urgent care handles is the single most expensive routine decision available in New York. The full comparison, including telehealth.
If you have insurance
Check that the site is in network, and remember the site and the clinician can be in different networks.
Ask what your urgent care cost sharing is. Many plans have a flat urgent care copay that is lower than the emergency room copay and higher than a primary care one.
If the site turns out to be hospital-affiliated, your emergency or outpatient hospital cost sharing may apply instead of the urgent care copay, which is the unpleasant surprise this page exists to prevent.
If you have no insurance
Two things stack, and most people use neither.
Ask for the self-pay rate, as above.
If the site bills through a hospital, New York’s financial assistance law reaches the bill: nothing out of pocket at or below 200% of the federal poverty level, capped at 10% of the Medicaid rate to 300% and 20% to 400%, no asset test, immigration status not considered, and applications accepted even after the bill goes to collections. How to apply.
And separately from both: a federally qualified health center charges on a sliding scale by income and many have same-day slots. For something that can wait until morning, that is usually cheaper than any urgent care. What to do without insurance.
Questions people ask
How much is urgent care without insurance?
There is no single price, and it depends more on whether the site is independent or hospital-affiliated than on what you are seen for. Ask for the self-pay visit rate and what tests add to it before you register.
Why was my urgent care bill as big as an emergency room bill?
Most likely the site was a hospital outpatient department or a freestanding emergency department rather than an urgent care center. They can look identical from the street.
What is a facility fee?
A charge for the place rather than the care, billed when the site is licensed as part of a hospital. The full definition.
Does insurance cover urgent care?
Generally yes, usually with a flat copay that sits between a primary care visit and an emergency room visit. Check that the site is in network.
Is urgent care cheaper than the emergency room?
Almost always, for things urgent care can handle. For an actual emergency the comparison does not apply and you should go to an emergency room.
What if I cannot pay the bill?
If a hospital billed it, apply for financial assistance under New York’s law. If an independent center billed it, ask for its hardship policy and a payment plan in writing.
