What starts over
Most plans in New York run on the calendar year, so January 1 is when the counters reset.
| What | What happens on January 1 |
|---|---|
| Deductible | Back to zero. Care that was free in December, because you had met the deductible, can cost the full allowed amount again until you meet it |
| Out-of-pocket maximum | Back to zero. A year of heavy care starts over |
| Drug list (formulary) | The new year’s list applies. Drugs can move tiers, leave the list, or pick up new approval rules |
| Network | The new year’s contracts apply. A doctor in network in December may not be in January |
| Part D cap (Medicare) | Resets, at $2,400 for 2027, up from $2,100 |
The practical effect: January is the most expensive month of the year for people with regular prescriptions and ongoing care. Plan for it rather than discovering it at the counter. What a deductible is · What counts toward the maximum.
The first-week checklist
- Confirm the coverage is active. A new marketplace plan starts only once the first premium is paid. Check your plan’s online account or call member services.
- Put the new cards in your wallet and give them to your pharmacy before your first January refill.
- Check every prescription against the new drug list, by name and dose. If one needs prior authorization now, ask the prescriber to start it this week.
- Call your doctors’ offices with the exact plan name if you changed plans, or if your plan’s network changed.
- If you use the Medicare Prescription Payment Plan and switched drug plans, opt in again with the new plan; participation does not follow you.
- Keep the December statements. They are your proof of what was paid last year, and of which doctor you were seeing, if you need a transition or an appeal.
When the pharmacy says no in January
A refusal in the first weeks of the year is usually a rule that changed on January 1: a new tier, a new prior authorization requirement, a step therapy rule, or a drug that left the list.
Before paying full price, ask three things. Is there a transition supply? Medicare drug plans provide temporary fills in the new year for people already taking a drug that changed status, and many other plans have transition rules too. Can the prescriber file an exception request? And is there a covered alternative the prescriber agrees with? When a drug is denied.
If you are in a Medicare Advantage plan and January shows the plan does not fit, you have one change between January 1 and March 31. The January window.
What else changes in January 2027
Tax credits for some immigrants. From January 1, 2027, marketplace premium tax credits are limited to citizens, green card holders, Cuban and Haitian entrants and citizens of the Compact of Free Association nations. The Essential Plan runs on the same federal money for those groups. What changes, what stays open.
Medicare for some non-citizens. On January 4, 2027, federal Medicare eligibility changes for non-citizens outside those groups. Their Medicaid in New York continues with state funds.
Medicaid work requirements. From January 2027, many adults 19 to 64 must show 80 hours a month of work, school, training or service, or an exemption. People already enrolled meet it at renewal, starting with renewals from March 1, 2027. Who must report and who is exempt.
What does not reset: New York’s rule that state-regulated plans may charge nothing for insulin continues, and Medicaid, the Essential Plan and Child Health Plus still take applications any month. Insulin in New York.
Questions people ask
Does my deductible reset on January 1?
For most plans, yes: plans in New York generally run on the calendar year. Check your plan documents if yours runs on a different year, as some employer plans do.
Why did my prescription cost more in January?
Usually because the deductible reset, or because the drug moved tiers or picked up a new approval rule on the new drug list. Ask about a transition supply and an exception before paying full price.
What is the Medicare Part D cap for 2027?
$2,400 in out-of-pocket costs for covered drugs. After that, covered drugs cost nothing for the rest of the year.
My new marketplace plan does not show as active. What happened?
A new plan starts only once the first premium is paid. Check your plan account, and call the insurer if you paid and it still shows inactive.
I am in a Medicare Advantage plan and January went badly. Can I switch?
Yes, once, between January 1 and March 31, including back to Original Medicare with a drug plan.
What changes for Medicaid in January 2027?
Work requirements begin for many adults 19 to 64, checked at renewals starting March 1, 2027, and some adults will renew every six months.
