What can a health plan community office do for you?

A community office is the insurer’s storefront. It handles the paperwork side of your coverage, in person and usually in several languages, without a phone tree.

EmblemHealth describes Neighborhood Care services as member services, enrollment help, connections to local resources such as food and housing, and free classes. It also says plainly that Neighborhood Care does not provide medical care. If your problem is with the Medicaid case itself, go to the city instead: HRA Medicaid offices by borough.

What should I bring to a community office?

Healthfirst publishes the most complete list, and it fits every carrier:

  1. Your plan ID card and, if you have one, your New York State Medicaid benefit card.
  2. Proof of identity and age, such as a birth certificate.
  3. Proof of citizenship or immigration status, if you are enrolling.
  4. Proof of income: recent pay stubs, Social Security, SSI, veterans’ or pension letters.
  5. Proof of address: a rent receipt, mortgage statement or mail with your address.
  6. For Medicare enrollment: your Medicare card, your current drug coverage details, and a list of your doctors and prescriptions.
  7. Any letter from the plan, NY State of Health or HRA that you want explained.

Bring the letter itself, not a photo of it. The case number and the deadline on it decide what the office can do today.

Office, phone or HRA: which one is right?

Your situationBest first stop
You want to join or switch to a specific carrierThat carrier’s community office or enrollment line
Your renewal packet arrived and you need help filling it inYour plan’s office, or a free navigator
HRA says your Medicaid case is closingHRA, by phone or at a Medicaid office
You want to compare carriers without a sales pitchA certified navigator, not a carrier office
You need a new ID card todayThe plan’s office, or the member portal

A carrier office will help you with its own plans and is staffed by that carrier’s own enrollment staff. That is the weak point of this route: nobody at a Fidelis office will tell you MetroPlus has your cardiologist. For a neutral comparison use a free navigator or the carrier comparison method.

What to say at the desk:

“I’m a member of [exact plan name]. I need help with [renewal / a new card / changing my doctor]. Can you do that here today, and what document number will I get to show it was done?”
