What are you getting with a Fidelis Care Essential Plan?
With a Fidelis Care Essential Plan you get the state’s Essential Plan, administered by Fidelis Care. New York sets the benefit package, so the premium is $0, there is no deductible, and the covered services match every other carrier: doctor and specialist visits, hospital and emergency care, prescriptions, lab work and imaging, mental health and substance use treatment, and preventive care with no cost sharing. Dental and vision are included for most enrollees; confirm with Fidelis Care.
Fidelis Care grew out of a New York nonprofit tradition and now operates as part of Centene, a large national managed care company. It is one of the largest participants in Medicaid, the Essential Plan and Child Health Plus. In practice that shows up as a statewide footprint, heavy participation in public programs, and the administrative style of a big organization rather than a neighborhood plan. More about Fidelis Care as a carrier.
Who is eligible, and where is the income line?
Fidelis Care does not decide who qualifies; the Essential Plan rules do. You need to be 19 to 64, live in New York State, be lawfully present, not qualify for Medicaid, and have household income at or below 200% of the federal poverty level.
| Household size | 200% of the poverty level, 2026 |
|---|---|
| 1 | $31,920 |
| 4 | $66,000 |
| Each additional person | adds $11,360 |
The 2026 guidelines start at $15,960 for one person plus $5,680 for each additional person, doubled for the 200% line. Income is household income before taxes, projected for the year you are applying for, and it is reduced by pre-tax retirement contributions, HSA contributions and deductible self-employment expenses. Below roughly 138% of the poverty level, most working-age adults land in Medicaid instead, which is broader coverage at the same $0. The full table by household size is on Essential Plan income limits.
Does Fidelis Care cost more than other carriers?
No. On the Essential Plan, Fidelis Care costs the same as every carrier: nothing a month. The premium that once applied to Essential Plan 1 was eliminated in 2021, so every numbered version has been $0 since.
The number you may see on a Fidelis card is one of the four current versions of the program; since the 200% to 250% tier ended on July 1, 2026, there are exactly four. Essential Plan 1 covers lawfully present adults above 150% up to 200% of the poverty level and is the only version with visit copays: $15 for primary care, $25 for a specialist, $75 for the emergency room and $150 for a hospital stay, capped at $360 a year. Essential Plan 2 covers incomes above the Medicaid line up to 150%, with $0 visits, $1 to $3 drug copays and a $200 cap. Essential Plan 3 (up to 138%) and Essential Plan 4 (up to 100%) are for lawfully residing immigrants who cannot get federally funded Medicaid; Plan 3 has only $1 to $3 drug copays and Plan 4 none at all. These are the 2026 amounts in EmblemHealth’s chart for the same state-set design. The application assigns the version from your income and status; you do not pick it. Essential Plan 1, 2, 3 and 4 compared.
Where money can differ between carriers is the drug list. Two carriers with identical premiums can differ by hundreds of dollars a year on one medication, depending on its formulary tier. That is a real cost question, and it is specific to your prescriptions.
What does statewide reach mean for a New York City member?
Statewide reach means Fidelis Care’s Essential Plan network spans the whole state, which is useful if you move around New York or live at the city’s edge. It does not mean the network is deep on your block. Reach and density are different measurements, and a carrier can be everywhere in the state while thin in one neighborhood’s specialist offices.
| If your situation is | What it suggests |
|---|---|
| You split time between the city and upstate | Fidelis Care’s statewide footprint is worth weighing |
| You live at the city’s edge | Worth checking alongside the city-centered carriers |
| Your care is inside NYC Health + Hospitals | Compare with MetroPlus, the public system’s own plan |
| Your specialist is in a large academic system | Check all four carriers; networks diverge most here |
| Your doctor already takes Fidelis Care | The strongest argument for it |
Treat each row as a starting point. One phone call to your own doctor outranks any of them. How the four carriers compare.
Which questions decide whether Fidelis Care is right for you?
Four questions decide it, and you ask them of the practice, not the directory. Directory accuracy is an industry-wide problem, not a Fidelis one, so every network answer needs a call.
- “Is my doctor in network for this exact plan?” Give the full Fidelis Care plan name. A practice can take one Fidelis product and not another.
- “Are they accepting new patients under it?” Being in network and having capacity are separate facts. The second decides whether you get an appointment this month.
- “Where is my prescription on the formulary?” For a maintenance medication, its tier is worth more than any comparison chart.
- “Which hospital would this plan send me to?” For a planned procedure this matters more than for routine care, and it is the question people skip.
- Save the member services number in your phone before you need it. The most expensive moment in this system is a front desk telling you your plan is not accepted, with no way to check who is right.
What happens if your income moves you to another program?
If your income crosses a line, your coverage changes programs, and staying with Fidelis Care can keep your doctors through the move. New York’s programs sit in a ladder by income: Medicaid, then the Essential Plan up to 200% of the poverty level, then marketplace plans with tax credits above it. Fidelis Care participates across several rungs.
The limit is that networks differ between a carrier’s Medicaid product and its marketplace product. Staying with the same company does not guarantee the same doctors, so check when a change is coming rather than discovering it at a front desk.
If your income rises above 200%, report it: you move to a marketplace plan with tax credits rather than losing coverage outright. If it drops, you may qualify for Medicaid. Until July 1, 2026, the Essential Plan reached 250% of the poverty level; that band ended when its federal funding did.
What are the honest limits of a Fidelis Care Essential Plan?
The first limit is density: a statewide network is not automatically a dense one where you live. The second is process. Prior authorization is standard across the industry, but administrative experience varies, and large organizations run more process. If you have had trouble with authorizations before, that is a fair thing to weigh.
The third limit applies to every carrier. If your doctor leaves the network mid-year, ask Fidelis Care about continuity-of-care provisions, which can cover a transition period for an ongoing course of treatment, and get the answer in writing.
None of these is a reason to avoid Fidelis Care. They are reasons to check before enrolling instead of after.
Questions people ask
Is the Fidelis Care Essential Plan free?
Yes, in premium: $0 a month and no deductible on every version. Essential Plan 1, for incomes above 150% of the poverty level, has visit copays capped at $360 a year; Essential Plan 2 and 3 have $0 visits and small drug copays; Essential Plan 4 has no copays.
Are Fidelis Care Essential Plan benefits different from Healthfirst’s?
No. The state sets the benefits for every carrier. What differs is the network, the drug list and the administrative experience. Healthfirst’s version follows the same rules.
Can I use a Fidelis Care Essential Plan outside New York City?
Fidelis Care operates statewide, which is its main advantage for people who move around New York. Confirm that the specific doctors you would use are in network.
Can I switch away from Fidelis Care mid-year?
Yes. The Essential Plan is a year-round program, so carrier changes are not tied to open enrollment.
What if my income goes over 200% of the poverty level?
Report it. You move to a marketplace plan with tax credits. Fidelis Care may offer one, but check your doctors again, because marketplace and Essential Plan networks can differ at the same carrier.
