What is the Healthfirst Essential Plan?

The Healthfirst Essential Plan is New York’s Essential Plan with Healthfirst’s name on the card. The Essential Plan is a state-run health plan for people who earn slightly too much for Medicaid, and the state sets its benefits, its $0 premium and its no-deductible rule. Healthfirst is one of the companies that delivers it, alongside Fidelis Care, MetroPlus, EmblemHealth and others.

Healthfirst itself was founded by a consortium of New York City hospitals and health systems, not by a national insurance company. That origin shapes its network: strong in the city’s community and safety-net hospitals, heavy in the public programs, and frequently the default carrier in the outer boroughs. It is one of the largest carriers for Medicaid managed care and the Essential Plan in New York City, which is why so many people searching for the program land on its name. More about Healthfirst as a carrier.

Who qualifies for the Healthfirst Essential Plan?

You qualify for the Healthfirst Essential Plan exactly as you would for any carrier’s version, because eligibility belongs to the program. Five conditions have to be true at once: you are 19 to 64, you live in New York State, you are lawfully present, you do not qualify for Medicaid, and your household income is at or below 200% of the federal poverty level.

200% of the federal poverty level, 2026 (48 contiguous states and D.C.)
Household size100% FPL200% FPL
1$15,960$31,920
2$21,640$43,280
3$27,320$54,640
4$33,000$66,000

Each additional person adds $5,680 to 100% FPL. Annual figures.

Those are 2026 figures, annual and before taxes: $31,920 for one person and $66,000 for a household of four, built from a base of $15,960 plus $5,680 for each additional person, doubled for the 200% line. Income means household income projected for the year, reduced by certain pre-tax deductions such as retirement and HSA contributions. If you are near the line, read what counts as income before deciding you are over it.

Until July 1, 2026, the program reached 250% of the poverty level. That upper band was eliminated when its federal funding ended. If your income is at or below 200%, the change did not affect you.

What does Healthfirst decide, and what does the state decide?

The state decides everything about cost and benefits; Healthfirst decides access. This split is the most useful thing to understand before choosing, because it tells you what to compare and what to ignore.

Covered services include doctor visits and specialists, hospital and emergency care, prescriptions, lab work and imaging, mental health and substance use treatment, and preventive care with no cost sharing. Dental and vision are included for most enrollees; confirm with Healthfirst. Choosing Healthfirst does not buy a better benefit, and choosing another carrier does not buy a worse one.

What do Essential Plan 1, 2, 3 and 4 mean on a Healthfirst card?

The numbers on a Healthfirst Essential Plan card are the four current versions of the program, not old labels. Since July 1, 2026, when the 200% to 250% tier ended, the Essential Plan has exactly four: Essential Plan 1, 2, 3 and 4. You do not choose a version: NY State of Health assigns it from your income and immigration status, and the state sets the cost sharing, so it is the same at every carrier.

VersionWho it is for (2026)Income ceiling, 1 personOut-of-pocket maxPrimary care / specialistEmergency room / hospital stay
Essential Plan 1Lawfully present adults 19 to 64, above 150% up to 200% of the poverty level$31,920$360$15 / $25$75 / $150
Essential Plan 2Lawfully present adults 19 to 64, above the Medicaid line up to 150%$23,940$200$0 / $0$0 / $0
Essential Plan 3Lawfully residing immigrants 21 to 64 up to 138% who cannot get federally funded Medicaid$22,025$0$0 / $0$0 / $0
Essential Plan 4Same group, up to 100%$15,960$0$0 / $0$0 / $0

All four cost $0 a month with no deductible. Only Essential Plan 1 has visit copays; Essential Plan 2 and 3 have $0 visits and $1 to $3 drug copays, and Essential Plan 4 has no copays at all, prescriptions included. These are the 2026 amounts as EmblemHealth publishes them for the same state benefit design. Essential Plan 3 and 4 are not Medicaid: they are the Essential Plan, run through the same carriers, for people whose immigration category keeps them out of federally funded Medicaid. Essential Plan 1, 2, 3 and 4 in full, with drug copays and income limits by household size.

How do you check that your doctor takes the Healthfirst Essential Plan?

Call the doctor’s office and use the exact plan name from Healthfirst, not the brand. A practice can take a carrier’s Medicaid product and not its marketplace product, so “Do you take Healthfirst?” produces confident wrong answers. Online directories are stale across the industry; the front desk knows.

  1. Write your list first: primary care doctor, every specialist from the last two years, your pharmacy, your prescriptions and the hospital you would choose.
  2. Call each doctor and ask two questions in one breath: “Are you in network for Healthfirst [exact plan name], and are you taking new patients under it?” Participation and an open panel are separate facts.
  3. Write down who answered and when. If the plan and the office disagree later, the dated note is what you have.
  4. Look up each medication on Healthfirst’s formulary and note the tier. This is where identical plans separate.
  5. Ask about the hospital. Provider-sponsored plans differ from national carriers most sharply at the hospital level, in both directions.

The full method, with the same checks across four carriers, is in how to compare NYC health plans in an hour. Budget the hour. It is the most valuable hour in the enrollment process.

When is Healthfirst the practical choice, and when is it not?

Healthfirst tends to fit when your care already happens in community hospitals and neighborhood practices rather than the large Manhattan academic systems, and when your current doctor already takes it, which is the strongest argument available. It also fits when you are moving from Medicaid to the Essential Plan and want to keep the same carrier, because staying can preserve your provider relationships through the transition.

Look at another carrier when your specialist sits inside an academic system that is not well represented in the Healthfirst network, when you manage a condition with a specialty drug that another formulary treats better, or when you have had unresolved authorization problems and want a different administrative counterparty.

One honest limit of this advice: these are tendencies. A tendency does not tell you whether your cardiologist is in. Only the phone call does.

How do you enroll or switch to the Healthfirst Essential Plan?

You apply through NY State of Health, the state marketplace, and choose Healthfirst as your carrier at the end. The same application screens you for Medicaid, the Essential Plan and Child Health Plus at once, so you do not pick the program; the application sorts you. Apply online at nystateofhealth.ny.gov, by phone at 1-855-355-5777, or in person with a certified navigator, who is free and earns no commission.

Have ready proof of identity, proof of New York residence and proof of income: pay stubs, an employer letter, a tax return, or a self-attestation if you are paid in cash. Coverage typically starts the first of the following month.

What changes in 2027 for Healthfirst Essential Plan members?

From January 1, 2027, federal funding for the Essential Plan is limited, among immigrants, to lawful permanent residents, certain Cuban and Haitian entrants and citizens of the Compact of Free Association nations. Refugees, asylees, people with Temporary Protected Status and other lawfully present categories fall outside it. Green card holders are not affected.

NY State of Health says it has notified affected enrollees directly, by letter, text or WhatsApp. If you are in one of those categories and have not seen a notice, call 1-855-355-5777, keep your contact information current, and do not drop coverage on your own. What changes for immigrants in 2027.

Questions people ask

Does the Healthfirst Essential Plan cost anything?

No monthly premium and no deductible, on every version. Essential Plan 1, for incomes above 150% of the poverty level, has copays such as $15 for primary care and $25 for a specialist, capped at $360 a year; Essential Plan 2 and 3 have $0 visit copays and small drug copays; Essential Plan 4 has none.

Is the Healthfirst Essential Plan better than another carrier’s?

Not in benefits, because the state sets them. It is better for you only if it covers your doctors, your hospital and your medications. Check those by the exact plan name.

Is the Healthfirst Essential Plan the same as Healthfirst Medicaid?

No. They are different programs run by the same company. Medicaid covers lower incomes and includes long-term care, which the Essential Plan does not. The application decides which program you are in.

Can I keep Healthfirst if my income changes?

Often yes, because Healthfirst participates across several programs. Networks can differ between a carrier’s Medicaid and marketplace products, so confirm your doctors during the transition rather than assuming.

What happens to my Healthfirst Essential Plan at 65?

The Essential Plan covers ages 19 to 64. At 65, the answer is Medicare.

Can I switch from Healthfirst to another carrier mid-year?

Yes. The Essential Plan is not tied to an enrollment season. Report the change through NY State of Health.
