What is an OTC card in a Medicare plan?
An over-the-counter (OTC) card is a prepaid benefit card that some private Medicare plans give members to buy approved health items without a prescription: pain relievers, cold medicine, toothpaste, bandages, reading glasses and similar products. The plan sets the list, the stores and the amount, and the money can only be spent on what the plan approves.
The benefit exists only in Medicare Advantage and dual-eligible special needs plans (D-SNPs). Original Medicare does not include one, and a Medigap policy does not add one. In New York some carriers also attach an OTC card to non-Medicare products; EmblemHealth’s Essential Plan card is the clearest example.
Three rules hold across every New York card we checked. The card is for the enrollee only, not for family members. It cannot be turned into cash. It cannot pay for Part B or Part D prescription drugs. Healthfirst and EmblemHealth both state these limits on their own OTC pages.
Which New York plans include an OTC card?
The first table lists each carrier’s own published plan list. Plan names change every January, so read the 2027 name on your Annual Notice of Change before relying on a row.
| Carrier | Card name | Plans the carrier lists with the card | Allowance period | Does unused money carry over? |
|---|---|---|---|---|
| Healthfirst (New York) | OTC Plus card | Life Improvement Plan (HMO D-SNP), CompleteCare (HMO D-SNP), Connection Plan (HMO D-SNP), Increased Benefits Plan (HMO), 65 Plus Plan (HMO) | Each benefit period | No, unused balances expire at the end of each month |
| EmblemHealth Medicare | OTC card | VIP Dual, VIP Dual Enhanced, VIP Dual Reserve (all HMO D-SNP): monthly. VIP Value (HMO-POS): quarterly | Month or quarter, by plan | No, not to the next month or quarter |
| EmblemHealth Essential Plan | EmblemHealth OTC card | Essential Plan members | $125 each calendar quarter, up to $500 a year (2026) | To the next quarter, but not past December 31 |
| Wellcare by Fidelis Care | Wellcare Spendables | 2027 list: Assist (002), Dual Align Unity (003), Patriot Simple (010), Dual Liberty Sync (014 and 015), Dual Access (016 and 017). Wellcare Fidelis Simple (004) has none | Loaded on the 1st of each month | Month to month, but expires December 31 |
EmblemHealth notes that OTC benefits vary by service area, and Healthfirst says the list of eligible items can change month to month. Neither carrier’s dollar amount is the same across plans.
How do I check my OTC card balance?
Use the channel your own carrier publishes. Third-party “OTC balance” websites and phone numbers that appear in search results are not run by the carriers, and some ask for your card number.
| Your card | Where the carrier says to go |
|---|---|
| Healthfirst OTC Plus | MyHFNY.org, the Healthfirst NY Mobile App, or 1-833-684-8472 (24 hours, 7 days). Member Services: 1-888-260-1010, TTY 1-888-542-3821, Monday to Friday 8am to 8pm |
| EmblemHealth Medicare OTC | EmblemHealth Medicare Connect Concierge, 877-344-7364 (TTY 711). Store and order lookup at mybenefitscenter.com; Convey orders at conveybenefits.com/emblemhealth or 855-858-5940 |
| EmblemHealth Essential Plan OTC | 888-682-2400 (TTY 711), 24/7, with your OTC card number and member ID; mybenefitscenter.com; or the OTC Network app |
| Wellcare Spendables (Fidelis) | The member portal (members.fideliscare.org), the mobile app, or the number on the back of the card. Activation: 833-647-9661 |
Before you go to the register, check the balance. Healthfirst and Wellcare both say that when the total is higher than what is left, you pay the difference with another payment method, and the card covers only approved items.
What to say on the phone:
“I’d like my current OTC balance, the date it reloads, and whether the store on [street] is a participating location for my plan. Is there anything I bought this month that was declined, and why?”
Healthfirst in New York vs Health First in Florida
These are two unrelated organizations with nearly the same name, and the mix-up costs people time every fall.
| Healthfirst (New York) | Health First (Florida) | |
|---|---|---|
| Where | New York City, Long Island and Hudson Valley counties | Brevard County, Florida |
| What it is | A New York health insurer founded by city hospitals, with Medicaid, Essential Plan and Medicare plans | A Florida health system with four hospitals that also sells its own Health First Health Plans |
| Website | healthfirst.org; members log in at MyHFNY.org | hf.org |
| OTC card | OTC Plus card; activation 1-833-684-8472 | Its own Medicare plan cards and rules |
If a website, a search result or a phone menu mentions Brevard, Melbourne or Florida, you are on the wrong Health First. A New York Healthfirst card works only with the New York company’s numbers, which are printed on the back of your member ID card. The same check applies to Convey: Healthfirst lists ConveyOTC among its online stores, but you reach it through MyHFNY.org or the app, not through a search result.
Where can I use the card?
Each carrier publishes its own store list, and the lists overlap only partly.
Healthfirst OTC Plus works at retailers such as Stop & Shop, Duane Reade, Walgreens, CVS, Walmart and Family Dollar, at many independent pharmacies, and at GrowNYC farmers’ markets for members eligible for the healthy food benefit. Online orders go through walmart.com, instacart.com, ConveyOTC or MyHFNY.org. The card carries a Visa logo but is not a debit card: if the terminal asks for a PIN, Healthfirst tells members to cancel and run it as “credit”.
EmblemHealth Medicare lists CVS, Duane Reade, Walgreens and Walmart, plus mail orders through Convey or CVS OTC Home Delivery. Mail orders should arrive within seven business days.
Wellcare Spendables works at participating retailers you find under “Shop In-Store” in the portal or app, and online under “Shop Online”. For eligible members it also covers some groceries, utilities and gas.
The extra uses: food, utilities, rent
Healthy food, utility bills and similar uses are not open to every member. Healthfirst says healthy foods and home utilities are special supplemental benefits for members of its D-SNPs who have a qualifying chronic condition such as diabetes, cardiovascular disease, lung disease or a disabling mental health condition, and that eligibility is not based on the condition alone. Fidelis likewise limits food, rent and utilities, gas, home safety items and pest control to eligible Wellcare members.
That distinction is where most disappointment comes from. A mailer may show groceries and an electric bill; the plan you join may give you only the OTC part. Ask before you switch:
“Is the food and utilities benefit included in my plan, or only for members who qualify? How do I find out if I qualify?”
Should an OTC card decide which plan you pick?
Rarely. An allowance of a few dozen dollars a month is small next to the network, the drug list and the cost sharing on a hospital stay, and the OTC money disappears at the end of each benefit period. Pick the plan that covers your doctors, your hospital and your prescriptions, then treat the card as a tiebreaker.
The weak spot in that advice: for a dual-eligible member who pays nothing for care, the OTC and food benefits may be the only real difference between two D-SNPs. In that case compare the allowances in each plan’s 2027 Summary of Benefits and Evidence of Coverage, which carriers publish for the new year, not a broker’s flyer. We do not quote 2027 dollar amounts on this page because they differ by plan and county.
