Do you need a Medicaid office at all?
Probably not, and checking first can save you a trip. New York City splits Medicaid between two systems, and only one of them has walk-in offices.
| Your situation | Where your case goes | Do you need an HRA office? |
|---|---|---|
| Under 65, applying on income | NY State of Health | No: apply online or at 1-855-355-5777 |
| Pregnant, or applying for children | NY State of Health | No |
| 65 or older | HRA | Optional: ACCESS HRA online works too |
| Disability or blindness | HRA | Optional |
| On Medicare, not a parent or caretaker of a minor child | HRA | Optional |
| Former foster youth under 26 | HRA | Optional |
| Need home care or long-term care | HRA | Optional; start with a call |
| Already on Medicaid and moved | Whichever system runs your case | No: update by phone, online or form |
The rule behind the table is HRA’s own: it accepts Medicaid applications from people 65 or over, people living with a disability or blindness, people receiving Medicare who are not a parent or caretaker relative of minor children, and former foster young adults under 26. Everyone else applies through NY State of Health. Why there are two doors.
HRA Medicaid office locations by borough

Medicaid offices by borough: detailed guides
Each borough guide below lists the HRA Medicaid offices plus the SNAP centers and Benefits Access Centers that share or sit near the same buildings, the documents to bring, interpreter rules, and the free state-funded enrollers that cover that borough. HRA’s addresses were checked again on November 3, 2026.
| Borough | Open HRA Medicaid offices | Also at HRA sites in the borough | Detailed guide |
|---|---|---|---|
| Brooklyn | Brooklyn South (495 Clermont Ave), East New York, Coney Island | 3 SNAP centers, including 2 George Street | Brooklyn Medicaid offices |
| Queens | Jamaica Community, Queens Community (Long Island City) | SNAP centers in Jamaica, LIC and Rockaway | Queens Medicaid offices |
| Bronx | Rider Community (305 Rider Ave), Fordham | SNAP centers in Crotona and Hunts Point | Bronx Medicaid offices |
| Manhattan and Staten Island | Chinatown, Dyckman Community, Staten Island (215 Bay St) | SNAP services in Washington Heights, East Harlem, 125th Street, 16th Street, 201 Bay Street | Manhattan and Staten Island Medicaid offices |
A SNAP center is not a Medicaid office, even in the same building. HRA lists Medicaid applications at its Medicaid offices and on ACCESS HRA, so check the floor before you join a line. If you plan to mail a paper application or a change form instead of visiting, the official form numbers and download links are on our New York Medicaid forms page.
How to apply without going to an office
HRA cases can be started online, and NY State of Health cases never needed an office in the first place.
- Check which system is yours with the table above. If you are under 65 and applying on income, stop here and apply with NY State of Health at nystateofhealth.ny.gov or 1-855-355-5777.
- For an HRA case, apply on ACCESS HRA, the city’s online benefits portal. HRA lists it as an application route alongside its offices.
- Gather copies, not originals, of proof of identity, New York City residence, income and immigration status. HRA says to submit copies.
- Include proof of Medicare if you are 65 or older. HRA requires it, and if you are not enrolled in Medicare it requires you to enroll as a condition of Medicaid eligibility.
- If a document is missing, apply anyway. HRA says the application is deferred so you can provide it later, rather than refused.
- Write down the date you submitted, and keep copies of everything. Timelines decide more of these cases than arguments do.

Changing your address or reporting a change without an office
For a case run by HRA, call the HRA Medicaid Helpline at 1-888-692-6116. HRA also accepts form MAP-751K for address and contact changes, and MAP-751W for changes that need supporting documents, both by fax to 917-639-0837. The state Health Department also lists HRA’s general line, (718) 557-1399, for New York City.
Keep the address current. A renewal notice sent to an old apartment is the most common way people lose Medicaid they still qualify for. How Medicaid renewals fail.
What changes on January 1, 2027 for HRA clients
If you get Medicaid and Cash Assistance from HRA, the two are becoming separate cases. HRA’s October 2026 FAQ says that, because of new federal rules, New York State is changing the way Medicaid is managed, and your Cash Assistance and Medicaid will be two separate cases.
Your Medicaid benefits continue unchanged for now. What changes is how you renew: you will get a notice from New York State of Health with instructions, and HRA says you do not need to take any action right now. You may receive a text first. What you should do is keep HRA updated on your mailing address, household and income through mybenefits.ny.gov, ACCESS HRA or an HRA Benefits Access Center, so the state’s notices reach you. Questions about the 2027 Medicaid work requirement go to the NY State of Health call center, 1-855-355-5777. Medicaid work requirements in 2027.
What to say on the phone to the HRA Medicaid Helpline or an office:
“I’m [age], I [have Medicare / have a disability / need home care], and I want to apply for Medicaid with HRA. Which office handles my borough, do I need an appointment, and can I apply on ACCESS HRA instead?”
If you are turned away
Being sent somewhere else is usually a routing decision, not a denial. If an office tells you to apply with NY State of Health, it is because your case belongs in the income-only system. If you get a written denial, read the reason: a missing document is not a decision on the merits, and any decision can be reviewed through a fair hearing. For the other numbers you might need, from NY State of Health to Medicare, see the phone directory.
