Manhattan and Staten Island HRA Medicaid offices: addresses and hours
These are the offices on HRA’s Medicaid locations page for both boroughs, checked on November 3, 2026. HRA publishes these hours and no appointment or walk-in rules, so phone the office first.
| Office | Borough | Address | Phone | Hours (HRA) |
|---|---|---|---|---|
| Chinatown | Manhattan | 115 Chrystie Street, 5th Floor, New York, NY 10002 | 212-274-4719 | Mon–Fri, 9:00 a.m.–5:00 p.m. |
| Dyckman Community | Manhattan | 4055 10th Avenue, Lower Level, New York, NY 10034 | 929-221-3244 | Mon–Fri, 8:30 a.m.–5:00 p.m. |
| Staten Island | Staten Island | 215 Bay Street, Staten Island, NY 10301 | 929-221-8823 or 8824 | Mon–Fri, 9:00 a.m.–5:00 p.m. |
“HRA Medicaid office Manhattan” means one of two places at opposite ends of the borough: Chinatown on Chrystie Street in Lower Manhattan, or Dyckman on 10th Avenue in Inwood. Pick by where you live, not by which one you heard of first. The Bronx, Brooklyn and Queens are on the citywide list of HRA Medicaid offices.
SNAP centers and Benefits Access Centers in both boroughs
HRA runs SNAP centers for food benefits and Benefits Access Centers (BACs) for Cash Assistance separately from its Medicaid offices. Several Manhattan sites are SNAP or BAC only, which is why people in Harlem sometimes end up in the wrong building.
| Site | Type | Address | Hours (HRA) |
|---|---|---|---|
| Washington Heights | SNAP center | 4055 10th Avenue, Lower Level, New York, NY 10034 | Mon–Fri, 8:30 a.m.–5:00 p.m. |
| East End | SNAP center and BAC | 2322 Third Avenue (SNAP on 3rd Fl.), New York, NY 10035 | Mon–Fri, 8:30 a.m.–5:00 p.m. |
| St. Nicholas | BAC with SNAP services | 132 W. 125th Street, New York, NY 10027 | Mon–Fri, 8:30 a.m.–5:00 p.m. |
| Lower Manhattan | BAC with SNAP services | 109 E. 16th Street, New York, NY 10003 | Mon–Fri, 8:30 a.m.–5:00 p.m. |
| Dyckman | BAC | 4055 10th Avenue, New York, NY 10034 | Mon–Fri, 8:30 a.m.–5:00 p.m. |
| Richmond (Staten Island) | SNAP center and BAC | 201 Bay Street (SNAP on 2nd Fl.), Staten Island, NY 10301 | Mon–Fri, 8:30 a.m.–5:00 p.m. |
On Staten Island, the Medicaid office is at 215 Bay Street and the SNAP center and BAC are a few numbers away at 201 Bay Street. In Inwood, Medicaid, SNAP and Cash Assistance all share 4055 10th Avenue, so ask for the Medicaid office on the Lower Level.
Who needs an HRA office and who applies with NY State of Health
HRA says it accepts Medicaid applications from people 65 or older, people with a disability or blindness, people on Medicare who are not parents or caretakers of minor children, and former foster youth under 26. Everyone else applies with NY State of Health, online or at 1-855-355-5777 (TTY 1-800-662-1220).
The state Health Department adds that more people outside the income-only rules now get Medicaid through NY State of Health, including people with both Medicare and Medicaid, people 65+ not seeking long-term care, and Medicare Savings Program applicants. Wherever you start, the state says staff will help you apply in the right place.
| If you are… | Start here |
|---|---|
| Under 65 and applying on income | NY State of Health |
| Pregnant, or applying for children | NY State of Health |
| 65+ and need home care or nursing home care | HRA: ACCESS HRA, Chinatown, Dyckman or Bay Street |
| On SSI, or in the Excess Income (spend-down) program | HRA |
| Disabled without a Social Security disability decision | HRA, which refers you for a state disability review |
Why New York splits Medicaid this way: where to apply for Medicaid in NYC.
Applying online instead of crossing the borough
HRA lists ACCESS HRA, at a069-access.nyc.gov/accesshra, as a way to apply for Medicaid online and to submit renewals. Staten Island has a single Medicaid office at 215 Bay Street, and Harlem has none, so for many people in both boroughs the nearest office is a long trip. Online saves it.
- Confirm HRA is your door with the table above.
- Copy your documents before you start. HRA asks for copies, not originals.
- File on ACCESS HRA and keep the confirmation with the date.
- Send what you have. HRA says an application missing a document is deferred, not refused.
- If you must go in, call the HRA Medicaid Helpline, 1-888-692-6116, and ask which office has your case.
To change your address or report a change, HRA takes forms MAP-751K and MAP-751W by fax. Both are linked from our Medicaid forms page.
What to bring to Chinatown, Dyckman or Bay Street
The state Health Department lists the documents HRA-type (non-MAGI) applicants need. Bring copies of what applies:
- Citizenship or immigration status. A U.S.-born citizen with a valid Social Security number is usually verified by match; a naturalized citizen brings a naturalization certificate or U.S. passport.
- Age, if Social Security cannot confirm it.
- Four weeks of recent pay stubs if you work.
- Other income: Social Security, VA benefits, pension, unemployment, child support.
- Where you live: rent receipt, landlord statement, mortgage statement or a recently mailed envelope.
- Medicare card and any other insurance card.
- Bank accounts, insurance policies and other resources if you are 65+, blind or disabled and applying for nursing home or community long-term care.
HRA requires proof of Medicare from applicants 65 and older and makes Medicare enrollment a condition of eligibility for those not enrolled. The state says it will tell you if more proof is needed to finish the case.
Language help at the Manhattan and Staten Island offices
HRA says it provides free interpretation at all walk-in sites in about 180 languages, and you can ask for an interpreter at any time. If no bilingual worker is free, staff use HRA’s telephone interpretation service. You can bring your own interpreter if that person is 18 or older, and HRA’s “I Speak” card lets you show your language at the desk.
The rule is the same at all three offices: the office name or neighborhood does not limit which language you can ask for. Ask at check-in, and do not let a minor interpret an interview about income and immigration status.
If you are refused, HRA takes language-access complaints through 311, anonymously and regardless of immigration status. More on interpreter rights in health care: language access.
“I need a free interpreter in [language]. I’m here to apply for Medicaid with HRA.”
Free in-person help in Manhattan and on Staten Island
The state funds Facilitated Enrollers for the Aged, Blind and Disabled, community agencies that help people 65+, blind or disabled apply for Medicaid, free and in person. The state lists these for the two boroughs:
| Agency | Phone (state list) | Area served |
|---|---|---|
| Harlem Independent Living Center | (212) 222-7122 | Manhattan |
| Urban Health Plan | (718) 589-2440 | Bronx, Manhattan, Queens |
| Jewish Community Center of Staten Island | (718) 475-5200 | All five boroughs |
| Make the Road NY | (866) 365-2724 | Brooklyn, Queens, Staten Island |
| Medicare Rights Center | (800) 333-4114 | All five boroughs |
| Charles B. Wang Community Center | (718) 587-1100 | All five boroughs |
| South Asian Council for Social Services | (718) 321-7929 ext. 200 | All five boroughs |
For income-based NY State of Health applications, a certified navigator is free and earns no commission: free navigators.
Neither HRA nor the state publishes wait times, so a phone call is the only check before you travel. Local context on getting to care is on the Chinatown, Inwood and St. George pages.
