What regular Medicaid covers

If you have Medicaid in New York and you are not in a HARP, you still have mental health coverage through your managed care plan or through fee-for-service Medicaid. That includes outpatient therapy, psychiatric evaluation and medication management, and substance use treatment.

The practical constraint is the same one everybody hits: finding a provider who takes Medicaid and has an opening. Medicaid pays less than commercial insurance, so fewer private practices participate. The places that do participate reliably are clinics licensed by the Office of Mental Health, hospital outpatient behavioral health departments, and federally qualified health centers. Those three are where to start, not private solo practices. The full method for building a call list.

What a HARP is, and what it is not

A HARP is not a service. It is a kind of Medicaid managed care plan built for adults with more serious behavioral health needs, and it comes with an enhanced benefit package that regular plans do not have.

Who it is for. Adults 21 and older who are eligible for Medicaid managed care and who meet the state’s behavioral health high-risk criteria.

What it adds. Behavioral Health Home and Community Based Services: a recovery-oriented set of supports meant to help someone with significant needs stay in the community rather than in a facility. Employment and education supports, peer supports, habilitation, family support and training, and short-term crisis respite sit in this category.

What comes with it. Anyone enrolled in a HARP has to be offered care management through a state-designated Health Home. A care manager is the person who decides, with you, which of those services actually apply. Skipping this step is the most common way people end up in a HARP and never use the part that makes it worth being in.

The pharmacy piece nobody explains

On April 1, 2023, New York moved the pharmacy benefit out of Medicaid managed care plans and into NYRx, the state’s own pharmacy program. For anyone taking a psychiatric medication this matters in a specific way.

Before: every plan had its own drug list and its own prior authorization rules, so changing plans could mean changing medication.

Now: one statewide list, one set of rules. Your plan card is still your plan card, but the pharmacy claim goes to NYRx.

The transition applies to mainstream managed care, HARPs and HIV special needs plans. It does not apply to managed long-term care plans, the Essential Plan or Child Health Plus, which kept pharmacy inside the plan. How NYRx works and what to do if a drug is denied.

If you are denied a service

Medicaid denials have their own route, and it is not the commercial appeal route.

For a service denied by a Medicaid managed care plan, the usual order is the plan’s internal appeal first, then a fair hearing with the state. The deadlines are unforgiving in one specific way: you have 60 days to request a fair hearing, but only about 10 days, before the change takes effect, to keep an existing service running while you wait, and you have to ask for aid continuing in those words. What this page will not tell you

We do not say which therapy is right for anyone, which medication to ask about, or what a diagnosis means. That is clinical territory and it belongs with a clinician. What we can tell you is what the program pays for, who decides, and what to do when the answer is no.

Questions people ask

Does New York Medicaid cover therapy?

Yes, including outpatient therapy, psychiatric evaluation, medication management and substance use treatment. There is no separate premium or visit fee for it.

What is a HARP?

A Health and Recovery Plan, a type of Medicaid managed care plan for adults 21 and older with serious mental illness or substance use disorder, with an enhanced package of home and community based services.

How do I sign up for a HARP?

You do not apply directly. The state identifies who meets its high-risk criteria and sends a letter. Enrollment then goes through New York Medicaid Choice at 1-855-355-5777 or NY State of Health.

Where do I actually find a provider who takes Medicaid?

Clinics licensed by the Office of Mental Health, hospital outpatient behavioral health departments and federally qualified health centers participate far more reliably than private solo practices.

Why did my pharmacy stop going through my plan?

Because of NYRx. Since April 1, 2023 the Medicaid pharmacy benefit runs through the state program rather than the managed care plan, with one statewide drug list.

My plan denied a behavioral health service. What now?

Usually the plan’s internal appeal first, then a fair hearing. If you already receive the service, ask for aid continuing within about 10 days so it does not stop while you wait.
