Ask for the price in writing when you book

Small practices sit two and three to a building here, and a cash price is often quoted out loud at the desk. A number said out loud cannot be compared with anything later. A written one can.

The right is federal. If you are uninsured, or insured and choosing not to use your coverage for a service, you are entitled to a good faith estimate of expected charges before scheduled care, and you do not have to explain why you want it. How the estimate works.

Ask when you book, while you can still call somewhere else. Asking the morning of leaves you nothing to compare.

What makes the estimate stick

Keep it. The whole mechanism depends on being able to put the estimate next to the bill that arrives later.

  1. Get the estimate in writing before the scheduled service, naming every provider expected to bill you.
  2. Save it with the appointment date, in a photo or a folder. Paper disappears; the comparison is what you need.
  3. Compare it to the final bill from each provider separately. The threshold is measured per provider, not against a combined total.
  4. If one bill exceeds that provider’s estimate by $400 or more, you may open the federal patient-provider dispute process. There is an administrative filing fee, refundable in defined circumstances.
  5. File within 120 days of receiving the bill. While the dispute is pending, the provider may not pursue collection on that amount.

If the bill came from a hospital rather than a private office, New York law usually matters more than the estimate: at or below 200% of the federal poverty level a hospital may not charge an uninsured patient out of pocket for medically necessary care, with no asset test and no consideration of immigration status. Apply first, dispute what survives. The state caps · Uninsured, start here.

Three things a cash quote does not include

The quote at the desk is almost always for the visit itself. Three other charges travel with it, and each is a separate question to ask before you agree to anything.

The lab. Blood work usually goes to an outside laboratory that bills you directly, on its own price list, weeks later. Ask which lab, and what the self-pay price is there.

The imaging. An X-ray or ultrasound can generate two charges: taking the image and reading it. The radiologist who reads it may be a different company entirely.

The place. Ask whether the office bills as a private practice or as a hospital outpatient department. The same fifteen-minute consultation billed the second way carries an added charge for the location. What a facility fee is.

Paying cash does not close any door

The fear is that a cash income, with no pay stubs and no direct deposit, means there is nothing to show and therefore nothing to apply for. New York’s rule runs the other way.

The state accepts an attestation of income — you state what you earn — checked against electronic data sources. Paper documents are requested only where what you state is not reasonably compatible with that data. Keep a written record of what you earn as you go; that is what makes an attestation credible. Paid in cash, and covered.

A community health center works the same way: the sliding scale runs on a self-declaration of household income, uninsured patients are routine there, and immigration status does not disqualify anyone.

Two other doors stay open all year. The Essential Plan costs $0 a month at or below 200% of the federal poverty level with no deductible and no enrollment season. What it covers. And NYC Care asks no immigration question and has no income cutoff for membership.

The interpreter, and who should not be one

You are entitled to a professional interpreter at no cost. Name the language you actually want — Cantonese, Mandarin, Fuzhounese, Toishanese — because “Chinese” is not a language a coordinator can book.

Telephonic interpretation is a legitimate answer, not a brush-off. A phone line carries far more languages than any front desk can staff, and for a less common dialect it is often the only way to get a trained interpreter in the room at all. Ask for it by name and ask that the request be noted in your chart.

A child should not interpret a clinical conversation. Not a consent form, not discharge instructions, not medication directions. It is not about how good their English is; it is about what a dropped word costs and who ends up carrying it. The same goes for the cash price you are discussing: a number you did not fully understand is a number you cannot dispute later. How the right works.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.

Questions people ask

Can I ask for a price before the appointment if I am paying cash?

Yes. Uninsured and self-pay patients are entitled to a written good faith estimate of expected charges before scheduled care, and asking for one requires no explanation.

When should I ask for the estimate?

When you book. Asking on the day of the appointment leaves no time to compare prices elsewhere and turns a routine request into a problem.

The bill came in much higher than the estimate. What can I do?

If one provider’s bill exceeds that provider’s estimate by $400 or more, you can open the federal patient-provider dispute process within 120 days of receiving the bill.

Why did the same visit cost more at one office than another?

Often because the office bills as a hospital outpatient department rather than a private practice, which adds a facility fee for the location.

Does paying cash mean I cannot get coverage?

No. New York accepts an attestation of income with electronic verification, and community health centers set their sliding scale from a self-declaration of household income.

The office offered an interpreter by phone. Is that acceptable?

Yes. Telephonic interpretation is a legitimate way to meet the requirement and usually covers more languages and dialects than the staff on site.
