What a sliding scale is
A federally qualified health center is required to charge patients on a scale based on household income and size. That is a condition of how it is funded, not a favour it extends to people who look like they need it.
Three things follow, and each one is the opposite of what people assume.
It is published. There is a written policy with income bands. You can ask to see it.
It applies to everyone under the threshold, insured or not. Having a high-deductible plan does not disqualify you from the scale on the portion you pay yourself.
It is not means-testing in the way people fear. What they need is an income figure and a household size. What sliding scale means in detail.
Why it beats piecemeal payment
The scale typically covers more than the visit. Labs drawn at the center, and in many cases medications dispensed or prescribed through the center’s pharmacy arrangements, fall under it too.
That matters because paying a private practice out of pocket almost never works that way: the visit is one charge, the lab bills you separately weeks later from a company you have never heard of, and the prescription is a third transaction at retail price. Why one blood draw produces three bills.
Becoming a patient at a health center converts three unpredictable transactions into one predictable one.
Two populations, one map
Crown Heights holds a large Caribbean community and a large Hasidic one in overlapping blocks, and the health care each uses has partly separate referral habits, partly shared institutions.
The practical effect is the same one that appears across Brooklyn: where you get sent for a procedure follows the network you entered, not the geography. Ask at a first visit: “If I needed a specialist or a procedure, where would you send me?”
Language access covers both: Haitian Creole, French, Spanish and Yiddish are all within the interpreter right, and phone interpretation carries far more languages than any front desk. How the right works.
If you have no coverage at all
Check eligibility first. Medicaid, the Essential Plan and Child Health Plus take applications in any month of the year, and New York’s limits are broader than most people assume. The ten-minute check.
NYC Care if insurance is out of reach: no immigration requirement, no income cutoff for membership, 1-646-NYC-CARE.
And if a hospital bill has already arrived: at or below 200% of the federal poverty level a New York hospital may not charge an uninsured patient out-of-pocket costs; up to 300% the cap is 10% of the Medicaid rate, up to 400% it is 20%. No asset test, immigration status not considered, applications accepted even after collections. The procedure.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.
Questions people ask
What is a sliding scale?
A published policy that sets the fee by household income and size. It is a condition of how community health centers are funded, not a discount granted case by case.
I am paid in cash. Can I still use it?
Yes. There is a self-declaration process for income when there are no pay stubs, and it exists precisely for this.
Does it cover more than the visit?
Usually labs drawn there, and often medications through the center’s arrangements. That is the main advantage over paying a private practice piecemeal.
Do they ask about immigration status?
Not as a condition of care. Neither does NYC Care, and the state’s hospital financial assistance law bars immigration status from being considered.
I have insurance with a big deductible. Does the scale help?
Ask. Having coverage does not automatically disqualify you from the scale on what you pay yourself.
