Which name is actually on your card
Ask someone here whether they have Medicaid and the answer is immediate. Ask which plan delivers it and the answer usually stops.
The second question is the one that decides which doctors you can see. Medicaid coverage in New York mostly reaches people through a managed care plan: a carrier under contract with the state, running its own network of doctors, clinics and hospitals.
If you never picked one, one was picked for you. Being assigned a plan is ordinary administration rather than a judgement, and nothing about it is fixed.
Three places hold the answer. The name printed on your benefit card, which is the plan and not the program. Your NY State of Health account. Or the member services number on the back of the card. A certified navigator will read the card with you at no cost and earns no commission for any answer. What a navigator does · How Medicaid works in New York.
The program sets the benefits, the plan sets the network
This one distinction explains most of the confusion about Medicaid here.
The program decides what is covered and what it costs you. The state sets benefits and cost sharing, so comparing carriers on copays gets you nowhere: they match because they were never the carrier’s to set.
The plan decides who you may see. Each carrier builds its own network, and that is essentially the whole of what changes from one card to the next. It settles which primary care office will take you, which specialists are reachable, and which hospital sits at the end of a referral.
So choosing a carrier is choosing a network. Nothing else about it moves.
You can change it, and the county sets the menu
You are not stuck with the plan you were assigned. Changing a Medicaid managed care plan is a normal transaction, not an appeal and not a favour.
What limits the choice is geography. Health plans in New York are licensed and sold county by county, and a carrier builds a separate network for each county it sells in. Manhattan is New York County, so your menu is the New York County list — not what a cousin in the Bronx or Queens is looking at, and not what an advert covering the whole city implies.
The state publishes that list rather than making you assemble it from carriers. Work in that order: what is sold where you live, then which of those plans include the people you already see. Ask when a change takes effect and do not book anything on the new network before that date. Which plans exist where you live.
The call that settles it
A directory is a lead list, not a schedule. The last step is always the practice itself, and it is two questions asked in one breath.
- Start with the state’s own tool, at
pndslookup.health.ny.gov, built on data plans must submit to the state. It gives you a starting list that is less wrong than a carrier directory. - Say the exact plan name from your card, never the insurer’s brand. A practice can take a carrier’s Medicaid product and not its marketplace one, and “do you take [carrier]?” produces a confident wrong answer.
- Ask both halves at once: “Do you take [exact plan name], and are you accepting new patients with that plan right now?” Participation and an open panel are separate facts.
- Ask which hospital they refer to. That tells you more about a future bill than the plan’s name does.
- Keep a dated log of offices that could not see you. It turns “nobody would take me” into a complaint with facts in it.
An in-network office can still generate an out-of-network charge, because networks attach to individual clinicians as well as to practices. The full verification method.
Child Health Plus and the Essential Plan work the same way
Same structure, different program: the state sets the benefits, a carrier runs the network, and the carrier question is a network question again.
Child Health Plus reaches considerably further up the income scale than parents expect, takes applications in any month with no enrollment season, and covers children regardless of immigration status. How it works.
The Essential Plan is $0 a month at or below 200% of the federal poverty level, carries no deductible, and also has no enrollment season. What it covers.
One benefit belongs to Medicaid specifically and is almost never offered unless you name it: transportation to covered appointments, at no cost. In New York City it is booked with the state transportation manager at 844-666-6270, at least three days ahead for a routine appointment, and you book the return leg in the same call. How to book it.
The letter that ends more coverage than income does
Coverage is lost to mail here more often than to money.
Medicaid renews, and the renewal notice goes to the address on file. If that address is stale, the packet lands at an old apartment, nobody returns it, and the case closes while the household still qualifies. People find out at a pharmacy counter, months later.
The plan matters in this too, because there are two records rather than one: your NY State of Health account, and the managed care plan whose card you carry. Update both the week you move, and the clinic where you are seen while you are at it.
Then learn your renewal month. It sits on the account, it does not move, and knowing it turns a surprise into a task. Renewals and how they fail.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.
Questions people ask
How do I find out which Medicaid plan I have?
The plan name is printed on your benefit card, and it is not the same word as “Medicaid”. Your NY State of Health account shows it too, and member services on the back of the card will confirm it. A certified navigator will read the card with you for free.
Do copays differ between Medicaid plans?
No. Benefits and cost sharing are set by the program, not by the carrier, which is why they match across cards. What differs between carriers is the network of doctors, clinics and hospitals you can use.
Can I change my Medicaid plan?
Yes. Changing plans is a routine transaction rather than an appeal. Ask when the change takes effect before you book anything with the new plan’s providers.
Why is a plan available in another borough and not here?
Because plans are licensed and sold county by county in New York, and a carrier builds a separate network for each county. Manhattan is New York County, so your options are the New York County list.
What exactly should I ask when I call a doctor’s office?
“Do you take [exact plan name], and are you accepting new patients with that plan right now?” Use the product name from your card rather than the insurer’s brand, and ask both halves in one breath.
Does Medicaid pay for a ride to my appointment?
Yes, at no cost for covered appointments. In New York City you book it with the state transportation manager at 844-666-6270, at least three days ahead for a routine visit, and you arrange the return trip in the same call.
