The layer that is close
The community health center presence here is the strongest asset the neighborhood has, and it is underused by people who assume a clinic that treats the uninsured must be a last resort.
A federally qualified health center charges on a sliding scale set by household income, sees uninsured patients as routine, and does not disqualify anyone for being paid in cash: there is a self-declaration process. Many have behavioral health and dental on site.
For primary care, chronic conditions, prenatal care and children’s care, this layer is closer and cheaper than anything else available. Search by ZIP at findahealthcenter.hrsa.gov, and see the registry table below.
The layer that is far, and the benefit nobody mentions
Specialists, imaging and procedures are mostly elsewhere. That turns a covered appointment into a two-hour round trip with a transfer, and for somebody with a mobility problem, a job with no flexibility, or two small children, that is the point at which the appointment quietly does not happen.
What to do when a referral is far away
Ask whether it can be done closer. Some specialty care has more than one site, and the referring office knows which.
Ask whether any of it can be telehealth. For a follow-up or a results conversation, often yes, and a clinician licensed in New York can see you from anywhere in the state.
Ask about the first appointment and the follow-up together. A course of care with four visits is four trips, and planning for that at the start is how you avoid abandoning it at visit two.
Arrange the ride when you book the appointment, not the night before.
If you have no coverage
Check eligibility first. Medicaid, the Essential Plan and Child Health Plus accept applications in any month of the year. New York’s limits are broader than most people assume, and immigration status disqualifies fewer people here than elsewhere. The ten-minute check.
NYC Care if insurance is out of reach: no immigration requirement, no income cutoff for membership, 1-646-NYC-CARE, interpreters on the line.
If a hospital bill has arrived: at or below 200% of the federal poverty level a New York hospital may not charge an uninsured patient out-of-pocket costs; up to 300% the cap is 10% of the Medicaid rate, up to 400% it is 20%. No asset test, immigration status not considered, applications accepted even after collections. The procedure.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.
Questions people ask
Does Medicaid pay for transportation to appointments?
Yes. Non-emergency transport to covered medical appointments is a Medicaid benefit at no cost, and it is rarely offered unless you ask for it by name.
Who do I call for it?
The state transportation manager: 844-666-6270 in New York City. Not your health plan.
How far ahead do I have to book?
At least three days for a routine appointment. Urgent trips and hospital discharges have a different track, and you have to mention it when you call.
Does it cover the ride home?
Yes, but it has to be arranged. If you only book the trip out, you can be left waiting.
What is a community health center?
A federally qualified health center: charges on a sliding scale by household income, sees uninsured patients as routine, and often has behavioral health and dental on site.
My specialist is far away. Any alternatives?
Ask whether the specialty has a closer site, whether any visits can be telehealth, and arrange transport at the time you book rather than the night before.
