What one hospital means in practice
It means less choosing and more preparing.
In a neighborhood with several hospitals, a bad experience or a refused application has an alternative. Here it usually does not, so the things worth doing are the ones done before a problem rather than after: knowing what the hospital must publish, knowing what the law caps, and knowing where the complaint goes when the answer is wrong.
The hospital is required to publish its financial assistance policy, an application, and a plain-language summary written to be understood at roughly a fifth-grade reading level — and to give you that summary on admission, at registration or at discharge, and with any bill. The state health department also publishes financial assistance information hospital by hospital, which is the fastest way to see what was filed. How to find any hospital’s policy.
The floor that applies here as everywhere
| Household income | The most a hospital may charge an uninsured patient |
|---|---|
| At or below 200% of the federal poverty level | Nothing out of pocket for medically necessary care |
| 201% to 300% | No more than 10% of the Medicaid rate |
| 301% to 400% | No more than 20% of the Medicaid rate |
No asset test, immigration status may not be considered, written decision in 30 days, no lawsuit below 400%, payment plans capped at 5% of monthly income with 2% interest, and 180 days before collections. Applications are accepted at any stage.
Every hospital licensed by the state must accept the New York State Uniform Hospital Financial Assistance Application. Ask for it by name. If an answer contradicts the law, the escalation is the Attorney General’s Health Care Bureau Helpline, 800-428-9071. How to apply.
The trip to the specialist
This is the barrier that is actually felt day to day: the referral is across the bay, the appointment is at 9 a.m., and the trip decides whether it happens.
Medicaid pays for non-emergency transportation to covered appointments at no cost. It is booked with the state transportation manager rather than with your health plan, at 844-666-6270 in New York City, at least three days ahead for a routine appointment. Urgent trips and hospital discharges run on a different track and you have to say so on the call.
Book the return trip at the same time. Booking only the outbound ride is the most common way people end up stranded. How Medicaid transportation works.
Two more things worth asking at the referral: whether the specialty has a closer site, and whether any of the visits can be done by telehealth. Both answers are sometimes yes and neither is volunteered.
ER or urgent care, when the ER is the near thing
The usual advice — use urgent care for minor things — assumes the two are equally close. Here the hospital is often the nearest option, which pushes people to the emergency department for things it is not built for, and produces the largest bills.
Worth knowing in advance rather than at 10 p.m.: which local sites handle stitches, sprains, infections and imaging, and what their hours are. Community health centers on the peninsula work on a sliding scale by household income and see uninsured patients as routine.
And the thing that does not change: for a real emergency, go to the emergency room. Federal law requires screening and stabilization regardless of coverage, and New York’s surprise-billing rules keep the argument over the amount between the provider and the plan. ER, urgent care or telehealth.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.
Questions people ask
How many hospitals are on the Rockaway peninsula?
One full-service acute care hospital, since the other closed in 2012. Everything else means crossing the bay.
Does living here affect my right to financial assistance?
Not adversely. Emergency care assistance reaches any New York State resident, and every part of the state must fall inside at least one hospital’s primary service area, so a refusal on geography is worth questioning.
How do I get to a specialist appointment off the peninsula?
Medicaid pays for non-emergency transportation to covered appointments at no cost. Book it with the state transportation manager at 844-666-6270, at least three days ahead, and book the return trip at the same time.
Should I go to the ER or urgent care?
For a real emergency, the emergency room, always. For minor injuries and infections, knowing in advance which local site handles what saves a large bill — the ER being nearest is not a reason to use it for everything.
What if I have no insurance at all?
Community health centers here charge on a sliding scale by household income and see uninsured patients as routine. Hospital financial assistance applies separately and is accepted at any stage.
What form do I need for financial assistance?
The New York State Uniform Hospital Financial Assistance Application. Every hospital licensed by the state must accept it.
