What a public hospital is, in practice
A public hospital is not a private hospital that happens to treat more uninsured people. It is a public institution with obligations written into what it is, and three of them matter to a household.
It does not turn people away for inability to pay. That is true of every hospital’s emergency department under federal law, and it is true of the public system more broadly as a matter of mission.
Its financial assistance exceeds the state minimum. New York’s law sets a floor: nothing out of pocket at or below 200% of the federal poverty level, 10% of the Medicaid rate to 300%, 20% to 400%. The public system offers free care at or below 200% and a sliding scale from 201% to 400%, and applies it as a system rather than hospital by hospital. How to apply.
It has a front door for people who are not in an emergency. That door is NYC Care, and it is the part people miss.
Living next to it does not mean using it for everything
Proximity to a large hospital creates a habit, and the habit is expensive for routine care.
A hospital outpatient department adds a facility fee for work an independent office or a community health center bills without one. For a check-up, a blood draw, a plain X-ray or an ongoing chronic condition, the local layer is usually cheaper and often faster.
The question at booking, anywhere: “Is this location a hospital outpatient department, and do you bill a facility fee?”
Community health centers in the area charge on a sliding scale by household income and see uninsured patients as routine. Search by ZIP at findahealthcenter.hrsa.gov. What a sliding scale means.
Language
Haitian Creole is a working language of the neighborhood, alongside French, Spanish and English.
The distinction that matters is the same everywhere: a staff member who speaks your language makes the visit less frightening, and it is not the same service as a professional interpreter. The gap appears on consent forms, in a diagnosis conversation and in discharge instructions.
“I would like a professional interpreter in Haitian Creole. Please note in my record that I asked.”
Say the specific language. Phone interpretation carries far more than any front desk, and it is free. How the right works.
If you have no coverage
Check eligibility first, because a lot of people here qualify and never applied. Medicaid, the Essential Plan and Child Health Plus take applications in any month of the year. The ten-minute check.
Then NYC Care, if insurance is genuinely out of reach.
And if a bill has already arrived, apply for financial assistance regardless of how old it is. Applications are accepted at any stage, including after the account has gone to collections, and immigration status may not be considered. The state law and the public system’s own policy.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.
Questions people ask
What is NYC Care?
A membership program that gives you a primary care doctor and access to the public hospital system. No immigration requirement, no income cutoff for membership, and what you pay is on a scale. Call 1-646-NYC-CARE.
Is NYC Care insurance?
No. It is a membership in the city’s public system. If you are eligible for actual coverage, that is generally better, which is why the first step is always to check eligibility.
Is the public system’s financial assistance better than the state minimum?
Yes. Free care at or below 200% of the federal poverty level and a sliding scale from 201% to 400%, applied across the system.
Should I use the hospital for routine care?
Usually not. A hospital outpatient department adds a facility fee for work a community health center or an independent office bills without one.
Can I get an interpreter in Haitian Creole?
Yes, at no cost, on request, including partway through a visit. Say the specific language and ask for it to be noted in your record.
I already have a large hospital bill. Is it too late?
No. Financial assistance applications are accepted at any stage, including after the bill has gone to collections.
