What the county actually decides
Insurers in New York apply to sell in specific counties, and a plan approved in Queens County is not automatically approved in Kings County or in Nassau. So the menu you see when you apply is built from one input: your address.
Two consequences here specifically.
Neighbours can have different options. Someone on the Queens side of the line and someone a few blocks into Bushwick may be shown different carriers, different plan names and different prices for what looks like the same coverage. Neither is a mistake.
Moving a short distance can change the menu. A move within the city that crosses a county line, and that gives you access to plans that were not available before, is a qualifying life event with a 60-day window. When a move opens a window.
Which plans exist in your county.
Before the carrier’s directory, the state’s
New York runs a Provider and Health Plan Look-Up Tool at pndslookup.health.ny.gov, built on data plans are required to submit. It is a better starting point than a carrier directory because it is the state’s record rather than the carrier’s marketing, and in a neighborhood where people naturally search across two boroughs it saves a round of wasted calls.
It does not replace the phone call. It produces a list that is less wrong.
The programs where none of this applies
The county question belongs to marketplace plans. Three programs work differently and are worth checking first, because if one of them fits, the entire comparison is over:
Medicaid, the Essential Plan and Child Health Plus have no enrollment season and accept applications in any month. The Essential Plan costs $0 a month at or below 200% of the federal poverty level, with no deductible.
Your county still matters for which managed care plans deliver those programs — but eligibility does not turn on it. The five-question check · The 2026 limits by household size.
One more thing worth checking before booking
A practice owned by a hospital system bills the same consultation as a hospital outpatient department, which adds a facility fee to the visit. It is invisible from the street and from the sign on the door.
One question at booking: “Is this billed as a doctor’s office or as a hospital outpatient department?” What a facility fee is.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.
Questions people ask
Why do my neighbours get different plan options?
Marketplace plans in New York are approved county by county, so an address on the other side of a county line can produce a different menu of carriers and plans.
Can I see a doctor in Brooklyn on a Queens plan?
Generally yes. Networks reach across the city and the county line does not stop at the exam room. Check the network by calling the practice with your exact plan name.
Does moving a few blocks change my coverage?
It can, if the move crosses a county line and makes different plans available. That is a qualifying life event with a 60-day window.
Which county am I in?
Ridgewood is in Queens County. Use the address you actually live at on the application; that is also what the marketplace verifies.
Is there a better directory than my insurer’s?
Start with the state’s Provider and Health Plan Look-Up Tool at pndslookup.health.ny.gov, built on data plans must submit. Then call to confirm.
Does the county matter for Medicaid or the Essential Plan?
Not for eligibility. Those programs have no enrollment season and accept applications in any month; the county affects which managed care plans deliver them.
