The situation this neighborhood actually has

Not “I am uninsured and turning 65”. More often: a job, a good plan, and no obvious reason to change anything.

That is precisely the case Medicare penalizes if it is handled by doing nothing, because the exception that lets you wait is narrower than people assume — and because it turns on a fact about the employer rather than about the plan.

What the penalty is

10% of the standard Part B premium for every full 12 months of delay, added to the premium permanently.

Not a one-time catch-up payment. Seven years late means paying 70% more every month for life. Part D carries its own late penalty on a separate formula, which is why people who delay drug coverage because they take no medication are surprised later.

The full timing, including what happens when the job ends.

The HSA detail that catches people

If you contribute to a health savings account, contributions must stop once any part of Medicare begins.

The trap is retroactivity: enrolling in Part A after 65 can make that coverage effective up to six months earlier, which removes HSA eligibility for those months after the fact and turns contributions already made into excess contributions with a penalty attached. The common practice is to stop contributing six months before filing. The exact handling is a question for a tax professional; the six months are the part to know about in advance.

Two New York programs worth checking anyway

The Medicare Savings Programs. They are assessed on income now, not on what a career paid. New York runs two tiers and both sit higher than most states use: QMB at 138% of the federal poverty level, which pays premiums and cost sharing, and QI at 186%, which pays the Part B premium — and for most people that arrives as a larger Social Security payment each month. How to apply.

EPIC, New York’s own prescription program for residents 65 and over enrolled in a Part D plan, which reaches income levels well above what people assume qualifies. What EPIC covers.

Neither has anything to do with having worked or not worked. Both are checked in an afternoon.

Only then, the choice everyone talks about

Original Medicare with a supplement, or a Medicare Advantage plan. It is a real decision with real trade-offs, and it is the last step rather than the first — after enrollment timing, after the savings programs, after the drug coverage.

One local consideration: whichever way you go, check that the specialists and the hospital you already use participate, by calling them rather than by reading a directory. Advantage or Original · The two-question call.

Free, unbiased counselling exists in New York and sells nothing. Use it before an agent. Medicare in New York.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.

Questions people ask

I am turning 65 and still working. Do I have to take Part B?

Only if the employer has fewer than 20 employees. At 20 or more, a current employer’s plan lets you delay Part B without penalty, and an 8-month special enrollment period opens when the job or coverage ends.

What happens if my employer is small?

Medicare becomes the primary payer. The employer plan pays second and pays as if you had Part B, so skipping it leaves the primary share unpaid rather than saving you money.

When exactly is my enrollment window?

Seven months: the three months before your birthday month, that month, and the three after. Enrolling before the birthday month avoids a gap.

How bad is the late penalty?

Ten percent of the Part B premium for each full 12 months of delay, added permanently. Seven years late means 70% more every month for life.

Can I keep contributing to my HSA?

Not once any part of Medicare begins, and Part A enrollment after 65 can be made effective up to six months retroactively, so many people stop contributing six months before filing.

Is there help with Medicare costs in New York?

Yes. The Medicare Savings Programs pay the Part B premium at 186% of the federal poverty level and premiums plus cost sharing at 138%, and EPIC helps with prescription costs for residents 65 and over.
