What actually ends coverage here
Income changes end some enrollments. Paperwork ends more of them.
The pattern repeats: a household moves within the neighborhood, the renewal packet goes to the previous apartment, nobody sees it, the renewal is not returned, and the case closes. The person finds out at a pharmacy counter or a front desk, months later, still eligible the entire time.
That is a solvable problem, and it is solvable in advance.
If it has already lapsed
The order that works, and the first step is the one people skip:
- Find out what actually happened. Closed for non-return of paperwork, or terminated on a finding about income or household? They are different problems with different fixes.
- If it closed for paperwork, send the renewal now. Reopening a recently closed case is routine, and in many situations coverage can be restored without starting over.
- Ask about retroactive coverage for the gap. Medicaid can cover medical bills incurred in the months before the application in defined circumstances, which matters if care happened while the case was closed.
- If a decision is wrong, request a fair hearing. Free, no lawyer required, and the request itself has a deadline printed on the notice. How a fair hearing works.
- Ask for aid to continue at the same time. If requested in time, coverage can carry on unchanged while the appeal is decided — and the window for that request is shorter than the window for the hearing itself.
What to do with a bill from the gap
A bill for care received while a case was closed is not automatically yours to pay.
Ask the provider to re-bill once coverage is restored, and say the date the coverage was reinstated to. Ask the hospital specifically whether you were screened for coverage for the date of service — the question that reopens a self-pay balance more often than any other.
And separately from any of it, hospital financial assistance is a statutory right that applies regardless: nothing out of pocket at or below 200% of the federal poverty level, applications accepted at any stage, including after collections. How it works.
Do not assume the income rule ended it
The income lines in New York sit higher than most people expect, and they move with household size. A raise that feels significant frequently does not cross anything.
And if it does cross the Medicaid line, what is usually on the other side is not “nothing”. It is the Essential Plan at $0 a month up to 200% of the federal poverty level, with no deductible and no enrollment season. The programs hand you between them; that handover is what reporting a change triggers. The 2026 limits by household size · The five-question check.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.
Questions people ask
I lost Medicaid but my income did not change. How?
Most commonly the renewal packet went to an old address and was never returned, and the case closed on paperwork rather than on eligibility.
Can I get it back?
Usually. Reopening a case that closed for non-return of paperwork is routine, and retroactive coverage for the gap is available in defined circumstances. Ask about both.
What is a fair hearing?
A free formal appeal of a decision about your benefits. No lawyer is required, and the deadline to request one is printed on the notice you received.
Can my coverage continue while I appeal?
If you request aid to continue in time, yes. That window is shorter than the window for the hearing itself, so ask for both in the same call.
What do I do about bills from the months I was uncovered?
Ask the provider to re-bill once coverage is restored, ask the hospital whether you were screened for coverage for the date of service, and apply for hospital financial assistance regardless.
What should I update to prevent this?
The address on file, first. Then a working phone number, and knowing which month your renewal falls in.
