One visit, several billing entities
A single episode of care at a large hospital does not produce one bill. It produces several, from businesses that have nothing to do with each other beyond the building.
The hospital bills for the facility. The physician group bills for the doctor who saw you, and its legal name is often not the hospital’s name. Anesthesia is usually a separate group with separate contracts. The pathologist who reads the specimen and the radiologist who reads the scan bill on their own, and you will probably never meet either. Send-out lab work is a fifth envelope.
Each of them has its own contract with your plan, or no contract at all. Out of network attaches to each biller separately, which is how a patient who checked carefully still ends up with an out-of-network bill.
One more line specific to this part of Manhattan: a practice owned by a hospital system bills a consultation the way a hospital outpatient department does, adding a charge for the place. What a facility fee is.
What to ask when you book
Ask before the date, not at the desk on the morning of. Scheduling staff can answer most of it, and what they cannot answer they can at least name.
- Ask which entities will bill for this visit. In those words. You want a list of names, not a reassurance that everything is covered.
- Ask for the physician group’s legal name, then ask whether that group participates in your plan.
- Ask who provides anesthesia, if anesthesia is involved, and whether that group is a separate business.
- Ask where specimens and imaging reads go, and where blood work is sent. Those two answers are the ones nobody volunteers.
- Verify each name with your plan yourself, by phone, and write down the date and the reference number. The two-question call.
- If you are uninsured or paying out of pocket, ask for a good faith estimate. How that works.
None of this is rude. Staff at these hospitals are asked it constantly.
What the surprise-billing law does and does not do
If an out-of-network clinician treated you at a facility that does participate, New York’s protections were written for exactly that situation.
Financial assistance applies at these hospitals too
Academic and specialty hospitals are not exempt. Every hospital licensed by New York State must accept the Uniform Hospital Financial Assistance Application, whatever the name on the building.
| Household income | The most an uninsured patient can be charged |
|---|---|
| At or below 200% of the federal poverty level | Nothing out of pocket for medically necessary care |
| 201% to 300% | No more than 10% of the Medicaid rate |
| 301% to 400% | No more than 20% of the Medicaid rate |
No asset test. Immigration status may not be considered. The decision must come in writing within 30 days, and applications are accepted at any stage, including after an account has gone to collections.
If you are insured, the provision people miss: out-of-pocket medical costs over twelve months exceeding 10% of gross income make you underinsured, and the same caps apply. How to apply · Finding a specific hospital’s own policy.
One approval does not cover every envelope
So keep the envelopes rather than the totals. For each one, find the full legal name of the entity, the account number and the date of service. Then ask the same two questions: does this entity have a financial assistance or hardship program, and what does it need from me.
Ask each of them for an itemized bill as well, line by line rather than a total. Duplicate lines and coding errors surface there, and asking costs nothing.
If a hospital or a group refuses to give you its written policy, or will not take an application, that is the Attorney General’s Health Care Bureau Helpline, 800-428-9071.
The one refusal that can be legitimate
Geography is the only ground other than income on which a hospital can turn down a financial assistance application, and it applies to non-emergency care only.
Emergency services: assistance reaches any resident of New York State. Where in the state you live is irrelevant.
Medically necessary non-emergency services: assistance reaches residents of the hospital’s primary service area. At specialty hospitals that draw patients from everywhere, that distinction is not academic.
The guardrails matter as much as the rule. Areas must be drawn so that every part of the state falls inside at least one hospital’s primary service area, and a hospital may not draw or alter its area to avoid underserved communities. Hospitals may also extend assistance beyond the minimum, and some publish policies covering all city residents.
Two questions when you are told you are outside the area: was this billed as emergency or non-emergency care, and which hospital’s area covers my address. What a primary service area is.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.
Questions people ask
The hospital is in my network. Why is one of the bills out of network?
Because being in network attaches to each biller separately. The hospital, the physician group, the anesthesia group and the doctors who read scans and specimens each contract with plans on their own, and one of them can be outside your network while the building is inside it.
Which entities bill separately after one hospital visit?
Commonly the hospital for the facility, the physician group for the doctor, the anesthesia group, the pathologist or radiologist who read your results, and the lab. Ask at booking which ones will bill for your visit.
Does New York’s surprise billing law cover ambulance bills?
Yes for ground ambulances, which federal law does not cover. New York’s protections also cover emergencies and out-of-network clinicians who treat you at a participating facility, with the dispute running between the provider and the plan.
Can I get financial assistance at a large academic hospital?
Yes. Every hospital licensed by New York State must accept the state’s uniform financial assistance application. There is no asset test, immigration status may not be considered, and the decision must come in writing within 30 days.
The hospital approved my application. Do the doctors’ bills go away too?
No. The approval covers the hospital’s own bill. The physician group, anesthesia and the readers decide separately, so each one needs its own request.
The hospital says I live outside its primary service area. Can it refuse?
For non-emergency care, that ground exists. For emergency care it does not: assistance reaches any New York State resident. Ask whether the care was billed as emergency or non-emergency, and which hospital’s area covers your address.
