Why this arrives here before it arrives elsewhere
This corner of the Bronx supplies a large share of the city’s home health workforce, which produces a specific and unfair asymmetry: households that know exactly how home care works professionally, and still hit the same wall arranging it for their own parent.
Knowing the job is not the same as knowing the eligibility process. The job is clinical and daily. The eligibility process is financial, and it runs through Medicaid.
What the four things actually are
People use “home care” for four different arrangements, and they have different rules:
Personal care — help with bathing, dressing, meals, moving around. This is what most families mean.
CDPAP, the Consumer Directed Personal Assistance Program — the same help, but the person receiving it hires, trains and directs their own aide, who may be a relative. How CDPAP works now.
Home health care — skilled nursing or therapy at home, ordered clinically, usually time-limited.
Managed long-term care — the plan through which ongoing services are delivered once someone needs care for an extended period. What managed long-term care is.
If the income is over the limit
This is where most families stop, and it is the point at which they should not.
New York allows a pooled income trust: income above the Medicaid limit is deposited into a trust administered by a non-profit and used to pay the person’s own living expenses, and the remainder no longer counts against eligibility. It is a legal arrangement with real rules, and it is also routine. How a pooled income trust works.
The alternative for some households is a spend-down, where medical expenses above the limit are applied before coverage begins. Which one fits depends on the numbers. Excess income and the spend-down.
Neither is something to work out alone. Free legal services organizations across the city handle exactly this, and they charge nothing.
The assessment is where the hours are decided
Hours are set by an evaluation of need, not by the family’s request. Two things improve that conversation:
Describe the worst day, not the average one. An assessment that happens on a good morning produces hours calibrated to a good morning.
Write down what actually happens in 24 hours before the assessor arrives: who helps with what, at what time, and what goes wrong when nobody is there. A written record is harder to underweight than a recollection.
If the hours come back too low, or are later cut, that is a decision with an appeal route — and asking for aid to continue in time keeps the current hours running while the appeal is decided. How a fair hearing works.
If the home care worker is you
A neighborhood that supplies this much of the city’s home health workforce contains a lot of people who arrange care professionally and have none themselves. Which employer you actually have — an agency, CDPAP, or a household directly — decides what you are owed, and the thresholds are written in hours. Coverage for home care workers.
The benefit that gets forgotten
Separate from home care and easy to miss: Medicaid pays for transportation to covered medical appointments, at no cost. It is booked with the state’s transportation manager rather than with a plan, and it is essentially never offered without being asked for. How to book it.

How to use this table. It tells you what kind of facility exists and where — not whether they take your plan or accept new patients. Those change constantly and only a phone call settles them. A federally qualified health center charges on a sliding scale by income; a hospital-owned urgent care may add a facility fee; a hospital bills through several entities at once.
Questions people ask
Does Medicare pay for home care?
Not for ongoing long-term care. In New York that is paid for through Medicaid, which is why the process starts with a Medicaid application rather than with a home care agency.
Can I be paid to care for my mother?
Through CDPAP, yes — an adult child, sibling or other relative can be the paid aide. A spouse cannot.
My parent’s income is above the Medicaid limit. Is that the end?
No. A pooled income trust lets someone over the limit qualify by depositing the excess income, and a spend-down is the alternative in some situations.
Who decides how many hours we get?
An assessment of need. Prepare for it: describe the worst day rather than the average one, and bring a written record of what a full 24 hours actually requires.
They cut the hours. Can I appeal?
Yes, through a fair hearing, and if you request aid to continue in time the current hours run while the appeal is decided.
Does Medicaid pay for rides to appointments?
Yes, at no cost, booked with the state transportation manager rather than with your plan. It is rarely offered unless you ask for it by name.
