$372.50 Lowest Plan G premium in NYC, per month (UnitedHealthcare, AARP program)

$299.00 Lowest Plan N premium in NYC, per month (UnitedHealthcare)
7 Insurers selling Plan G in the NYC rate region; 5 sell Plan N

Medicare supplement plans are sold by private insurers, but their benefits are standardized: a Plan G from one company pays the same bills as a Plan G from another. That makes Medigap one of the few insurance products where comparing price is the whole comparison. New York’s Department of Financial Services (DFS) publishes every insurer’s monthly premium by region. We took the table for the NYC PROPER region, which covers ZIP codes starting 100 to 104 and 111 to 114, and checked each number against a second DFS file.

Over a year the cheapest Plan G costs $4,470 and the most expensive $10,083. Nothing in the policy explains the difference. Insurers price on their own claims experience, expenses and the business they want, and DFS approves each insurer’s rates separately.

Monthly premiums in the NYC PROPER region. “Not offered” means the insurer has no rate for that plan in this region. Globe Life shows two rates for New York City in every plan; DFS notes that a premium shown in a region “may be offered in a part or all of the region,” so ask Globe which applies to your ZIP code. Plans C, F and high-deductible F, which appear in the CSV, are sold only to people who became eligible for Medicare before January 1, 2020.

Plan G or Plan N: what the difference buys

Both plans leave you the Medicare Part B deductible, $283 in 2026, so it drops out of the comparison. What Plan N adds, according to DFS’s description of the plans:

  • a copay of up to $20 for each doctor visit;
  • a copay of up to $50 for each emergency room visit, waived if you are admitted;
  • Part B excess charges, the amount a doctor who does not accept Medicare’s rate can bill above it; Plan G pays those, Plan N does not.

So the question is simple arithmetic: does the yearly premium difference exceed what the copays would cost you? At UnitedHealthcare, G costs $882 a year more than N, the price of 44 office copays. At EmblemHealth the gap is $1,408 a year, 70 copays. At Humana it is $2,481 a year and at Bankers Conseco $3,801. At Globe Life the gap is so small, $132 or $144 a year, that N saves almost nothing.

Two findings matter more than the letter you pick. The insurer matters more than the plan. Bankers Conseco’s Plan N, at $523.54, costs more than Plan G at UnitedHealthcare, Aetna, EmblemHealth, Globe Life and Mutual of Omaha. And high-deductible Plan G costs $67.69 to $106.34 a month here, but you pay a yearly deductible set by Medicare before it pays anything; it suits people who want catastrophic protection and can absorb that deductible.

New York’s rules: community rating and year-round sales

Most states let a Medigap insurer price by age and refuse applicants with health problems once the first six-month window after Part B enrollment has passed. New York does not. DFS states that any insurer writing Medigap must accept an application at any time of the year, and may not deny a policy or set a different premium because of health status, claims experience or medical condition. The DFS tables are titled “community rated”: everyone in a region pays the same price for the same plan at the same insurer. The same protections apply to people on Medicare because of a disability or end-stage renal disease.

The one limit is a waiting period of up to six months for pre-existing conditions. DFS says it must be reduced by your earlier creditable coverage, including a Medigap policy or a Medicare Advantage plan, as long as there was no break of more than 63 days. Someone switching from one Medigap insurer to another usually has no wait at all. How this changes the Advantage-or-Original decision.

City versus suburbs and upstate. For every insurer, the NYC PROPER Plan G rate equals the Long Island rate; Globe Life also lists a second, higher city rate. Upstate can be cheaper: Aetna’s Plan G in the Buffalo region is $264.80 or $293.07, against $406.26 in the city.

What may change in 2027

Rates on this page are for 2026. DFS posted the 2027 changes insurers requested in the individual Medigap market on July 10, 2026, and said they had not been approved: Bankers Conseco +20.7%, Humana +21.3%, EmblemHealth +14.7%, Globe Life +11.0%, Mutual of Omaha +2.8%, a weighted average of 9.5% for all individual filers. Aetna and UnitedHealthcare do not appear in that individual-market list. Requests are usually cut; check the DFS rate tables again before you decide.

What this means for you

  1. Look up your own rate. Use the DFS Medicare supplement rate look-up on the DFS Portal with your ZIP code, or call the insurer.
  2. If you already have Plan G or N, compare your premium with the table above. Because New York sells Medigap all year without health questions, moving to a cheaper insurer for the same plan letter is open to you in any month.
  3. Apply for the new policy first, with a start date, and cancel the old one only after the new one is confirmed, so there is no gap.
  4. Get free, unbiased help from HIICAP, the state’s Medicare counseling program: 1-800-701-0501.

The sentence for the insurer: “I live in ZIP code [your ZIP]. What is my monthly premium for Plan G and Plan N, has a rate change been filed for 2027, and is there any waiting period if I’m switching from another Medigap policy?”

If your income is limited, check Medicare Savings Programs first: they can pay the Part B premium. And if you have Medicaid, a Medigap policy is the wrong product; Medicaid already fills those gaps.

Methodology

On October 16, 2026 we rendered the DFS page “Medicare Supplement Insurance Rate Plans,” which shows monthly premiums for August 1, 2026 by insurer and region, in a browser and extracted the NYC PROPER column for every plan with Python. We then downloaded the DFS PDF “Comparison of Year 2026 Community Rated Standardized Medicare Supplement Monthly Premiums,” in effect October 1, 2026, and extracted the same column by word position. For Plans A, G, N and high-deductible G every NYC value matches between the two files. The only difference we found is an EmblemHealth Plan C rate that appears in the PDF but not on the web page; Plan C is not part of this comparison. Annual figures are monthly premiums times 12, rounded to the dollar; “copays to offset” divides the annual G-minus-N gap by $20. The requested 2027 changes come from the DFS Portal page dated 07/10/2026. Group policies and Medicare Select policies are not included.

Questions people ask

How much does Medigap Plan G cost in New York City?

In 2026, from $372.50 a month at UnitedHealthcare (AARP program) to $840.28 at Bankers Conseco, across seven insurers. The benefits are identical, so the cheapest Plan G pays the same bills as the most expensive.

Is Plan G or Plan N better in New York?

It depends on how often you see doctors. Plan N costs less per month but adds copays of up to $20 per office visit and $50 per ER visit, and does not pay Part B excess charges. At UnitedHealthcare, Plan G costs $73.50 a month more than N; at Globe Life the gap is only $11.00.

Does Medigap cost more as you get older in New York?

No. New York requires community rating: insurers may not set premiums by health status, and the DFS tables show one price per plan, insurer and region, not one price per age. Rates can still rise for everyone when DFS approves an insurer’s yearly increase.

Can I switch Medigap plans any time in New York?

Yes. New York law requires insurers to accept Medigap applications at any time of the year, regardless of health. A waiting period of up to six months for pre-existing conditions can apply, reduced by your prior coverage.

Is a Medigap policy in Long Island the same price as in NYC?

For Plan G, yes at every insurer in the 2026 DFS tables: the NYC PROPER and Long Island rates match. Globe Life also lists a second, higher rate for the city.
